Conditions

The Illinois Department of Health has approved the medical use of cannabis for a list of 38 debilitating medical conditions that include Cancer, HIV, Glaucoma, ALS, MS, and Spinal Cord Injury. Patients are required to have a doctor verify one of these conditions and complete this form as part of their application.

There is a supplemental list of 11 conditions that are being reviewed by the Department of Health . It is important to note that the state is not yet accepting applications from patients who are diagnosed with these conditions.
